A home first.
Clinical care, quietly built in.

Care Smith is a nurse-owned and operated residence for adults with developmental disabilities. Daily life here looks like home: shared meals, real routines, neighbors and community. Behind it sits a Doctor of Nursing Practice who notices the small health changes before they become big ones.

A different standard of residential care

Where clinical rigor meets residential warmth.

Most group homes are run by care managers who call a nurse when something looks wrong. At Care Smith the nurse is the one running the home. That changes what gets noticed: a change in appetite, a new medication's side effects, a resident who is quieter than usual. Health changes are recognized and addressed with the precision of a trained clinician.

The rest of the day is deliberately ordinary. Residents help plan meals and cook them. They keep their own rooms, run errands in town, go to church or the park or the library. Belonging is treated as a health outcome, not an optional activity.

"Not just to live, but to thrive."The CareWELL commitment

Meet the founder

Dr. Suzanne Levy built the home she wished existed.

DNPAGPCNP-BCONC14 years in healthcare

Suzanne Levy is a doctoral-prepared, board-certified Adult-Gerontology Nurse Practitioner. Across 14 years in healthcare she has cared for adults with diverse medical, behavioral and support needs in a wide range of clinical settings.

Her work with adults with developmental disabilities began early, as a Direct Support Professional in a residential group home in Yonkers, New York. There she saw firsthand how compassionate support, meaningful relationships and community inclusion help people build confidence, gain independence and lead fulfilling lives.

Care Smith is the result: a nurse-owned and operated residence dedicated to wellness, inclusion and personal growth, where residents build meaningful connections and pursue lives filled with purpose and possibility.

Holistic wellness, every day

Health is what happens between appointments.

Medication and monitoring are the floor, not the ceiling. The CareWELL™ day is built around the things that keep a body and a mind well: movement, real food, rest, time outdoors and something to tend.

Rolled yoga mat and water bottle by an open door to a sunrise lanai
MoveDaily activity matched to each resident: walks, stretching, the pool at the Y, gardening. Physical activity is on the schedule, not left to chance.
Herbal tea steeping beside an open journal and reading glasses on a windowsill
Rest & nourishA dietitian-approved menu cooked at home, consistent sleep routines and quiet time, because rest and nutrition are treated as clinical inputs.
Hands tending basil and tomato plants in a raised garden bed at golden hour
Grow & belongA garden to tend, a congregation, a volunteer shift, a library card. Purpose and community are part of the wellness plan, and preventive care is scheduled around them.

The residence

A real house on a real street.

Care Smith is a single-family home in a Spring Hill neighborhood. Private bedrooms, a shared kitchen where meals actually get cooked, and a lanai for the part of Florida evenings that are made for sitting outside.
