Placement
An inquiry, a conversation, a visit, and a written plan before move-in. Dr. Levy answers the first message herself, and the person and family set the pace from there.

The process
The only clock in this process is ours: Dr. Levy answers the first message within about one business day. After that, the person and the family set the pace.
Send a note through the contact form or email info@caresmithcorp.com. A sentence or two is enough. Dr. Suzanne Levy reads it herself and replies personally, usually within one business day. You will not be passed to an intake line.
A phone or video call with Dr. Levy. She asks about health history, support needs, daily routines, and what a good day looks like for the person. Parents, guardians, and support coordinators are all welcome on the call. Funding and next steps are walked through here. If the fit is not right, this is where we say so.
Family, guardians, and support coordinators tour the house in Spring Hill and meet the team. The person who may live here is welcome to come. Ask to see the bedroom, the kitchen, the lanai. Come back a second time if that helps.
Before move-in, Dr. Levy writes a person-centered plan covering clinical oversight, behavior support, daily routine, and community goals. You read it and shape it. Move-in is scheduled around the person, not around a calendar.

Who we serve
Care Smith is a home for adults with developmental disabilities, licensed for up to six residents. Because the nurse is the one running the home, we can support individuals whose medical needs have made other placements difficult: several medications, chronic conditions that need active monitoring, health that changes from week to week.
We are not the right home for everyone, and we will not pretend to be. If the conversation tells us the person would be better served somewhere else, we tell you that, and we tell you why.

Before the call
None of this is required to start, and you do not need to send records before we talk. It simply makes the conversation more useful.

If the person has a written plan, have it nearby. If not, tell us what has worked and what has not. That is just as useful to us.
How they like to wake up. What they eat. What settles them when something goes wrong. Who they want to call. This is the part we listen to most closely.

When you call, you reach Dr. Levy, a doctoral-prepared, board-certified nurse practitioner, not a front desk. Send what you have and it will be read before the conversation.

No. Tell us what you know. If you have a medication list or a support plan handy, it helps, but the first conversation is about the person, not the paperwork. Records come later, when it is clear we are a fit.
Yes. Many families want a first look on their own and a second visit with the person who may live here. Come at a mealtime if you want to see the kitchen in use.
Dr. Suzanne Levy, the founder and clinical administrator, and the Registered Nurse who serves as back-up operator. You will also meet the team who will be in the home day to day.
Slowly, and around the person. The placement plan is finished before move-in, so routines, medications, and behavior support are known on day one. Short visits before the move help. Familiar bedding and belongings are welcome in the private bedroom.
We tell you, usually during the conversation, and we tell you why. We would rather you spend your time on a home that can serve the person well than on a placement that is not right for them.
